Collection of this information is authorized under 42 U.S.C. 241. The primary use of this information is to identify candidates for the Start-Up Challenge Program. This information may be disclosed to researchers for research training purposes and in response to Congressional inquiries. Submission of this information is voluntary, however, in order for us to accept you as a Start-Up Challenge recipient, you should complete all fields.

Submissions are due by 11:59 pm ET on April 20, 2023. Please review the full challenge announcement here for submission rules and instructions. By entering this submission, you are agreeing that you have reviewed the full challenge announcement, and that you agree to the terms and conditions of this challenge. Do not include any confidential or proprietary information in your submission.

You MUST ACKNOWLEDGE each of these statements regarding Challenge rules and expectations.

1. I am proposing a technology or product aligned with the NIA small business research priorities.

2. I have not been a finalist in any previous version of the NIA Start-up Challenge.

3. I am not a principal investigator on an NIH SBIR/STTR award that will be active on April 20, 2023.

4. I have formed a U.S.-owned and -operated small business or intend to form one by December 31st, 2023 in order to meet the eligibility requirements to apply for NIH SBIR/STTR funding.

5. If selected as a finalist, I am expected to commit up to 4-6 hours per week for approximately five months to participate in an accelerator program from approximately July 2023 – December 2023. Note: To provide maximum flexibility for participants, the program will be mostly virtual except for one or two in-person networking events (travel costs paid by NIH for at least one finalist per team).
